Jurista de profissão, Botha foi presidente do Partido Nacional Sul-Africano. Ocupou várias pastas de ministro, entre as quais a da Defesa sob a presidência de F. Verwoerd, entre 1966 e 1978. Em 1978 tornou-se primeiro-ministro e em 1984, graças a uma emenda constitucional, presidente. Entrou em conflito com os grupos bôeres ultraconservadores, dando os primeiros os para o desmantelamento do apartheid com a issão para o governo de representantes de grupos raciais não brancos, ainda que em ministérios de pequena relevância. Em 1989, introduziu uma reforma constitucional que previa uma participação política restrita desses grupos. Foi substituído no cargo de presidente por Frederick Willem de Klerk.
